- Our office participates with most of the major insurance companies.
- Patient’s also have responsibilities with these insurance companies.
- Patients are responsible for understanding the type of benefits and coverage of their policy
- Patients are required to pay their co-payment at the time of service.
- Patients are expected to bring their insurance cards to each visit. If a patient comes to the office without their card, they will be responsible for payment in full at the time of service.
- Patients are responsible for informing our office of any insurance change.
- Patients are responsible for obtaining their referrals from the primary care physician.
OUR OFFICE ACCEPTS RESPONSIBILITY WITH INSURANCE COMPANIES
- We will file all charges to your insurance company, with the exception of non-covered services, which you will be notified of at the time of service.
- We will file your secondary coverage, after the primary carrier has paid.
- If the patient fails to notify us of an insurance change, we cannot be responsible for non-payment.
- We will not be a part of or enter into any dispute among parents who are divorced or legally separated. Payment will be expected from the party bringing the child.
